Our services.

We’re dedicated to providing meaningful support that makes a difference. We focus on community connections and cultivating a sustainable structure within the home, to support you in living a fulfilling life.

Our project is uniquely peer-led and values lived experience. When you are a part of Project Home Unity, you know you will be understood, respected, and empowered by your supports.

  • We support anyone who will benefit from Queer-affirming social support! Our services are available to NDIS participants and private clients.

All ages are welcome. While we primarily support adults, we have extensive experience supporting young children and adolescents too.

  • We charge 90% of the price limit set out in the NDIS Pricing Arrangements and Price Limits. This allows participants to have a buffer in their budget to account for Activity Based Transport, report writing and other Non-Face-To-Face supports, and changes to their support needs.

If you are unable to afford our rates for any reason (including insufficient funding), please contact us to discuss alternative pricing arrangements.

  • Yes we do!

If you are eligible to access the NDIS, we are able to guide you through the process, and support you in gathering evidence and submitting your access request.

Social Support

Social support refers to the physical and emotional support provided by your social network. This could be your friends, family, co-workers, or larger community. Having consistent, reliable social support is essential for maintaining mental health, coping with stress, and building resilience!

At Project Home Unity, we integrate with your social network - we are here to supplement and strengthen your informal supports. We fill in the gaps where informal supports are unavailable or unsustainable. We also work together with your primary supports (where appropriate) to inform our service delivery and strengthen your relationships!

Some ways a PHU Social Support Worker can support you in your daily life include:

  • Self-care assistance

  • Support to attend appointments

  • Going out and attending social and community events

  • Helping with household and domestic tasks like shopping, cooking, walking the dog, cleaning and organising, personal admin, and more.

Support Coordination

Support Coordination is designed to assist you in accessing a mix of support services that will help you achieve your goals.

A Support Coordinator works with you to decide which supports you would like to implement, providers that will meet your needs, and design a schedule of supports that reflects your goals and fits your budget. They also report back to the NDIA about your plan implementation, progress, and reassessment (plan review).

At Project Home Unity, we offer Level 1 & 2 Support Coordination and Psychosocial Recovery Coaching. These services are all under the “Support Coordination” umbrella, and differ slightly in their delivery.

Skills Development

“Life skills” simply refers to the skills you need to make the most out of your life. This is incredibly personal and linked to your current skills, capacity, and goals!

“Life skills” can include (but are certainly not limited to):

  • Planning and organisation

  • Social and communication skills

  • Budgeting and financial planning

  • Housing and tenancy assistance

  • Transport training

  • Cooking and meal planning

  • Training in Self-Management (for Self-Managed NDIS participants)

Our skills trainers take a strengths-based approach to skills training, working with you and finding creative alternatives where needed.

Community Building

Coming Soon!

We’re stronger together! Research shows that community resilience greatly improves both mental and physical health. We’re currently organising inclusive peer-led community groups for LGBTQI+ individuals to cultivate supportive social networks, create safe spaces, and provide inclusive education.

These groups aim to provide marginalised and minority groups with a welcoming and comfortable space to connect to one another, meet people with similar life experience, and learn from one another and community leaders.
